Brian DeFade, D.O. is a graduate of the West Virginia School of Osteopathic Medicine in Lewisburg, W.Va. He completed a general surgery internship at Charleston Area Medical Center (CAMC) in Charleston, W.Va. Dr. DeFade completed a urology residency at CAMC where he was chief resident. He also recently completed a urological specialty fellowship in erectile dysfunction and voiding dysfunction at the University of North Carolina/Chapel Hill in Chapel Hill, N.C.

In order to provide the best possible care, please remember to bring your insurance card and a list of any medication you are currently taking to your appointment.
Insurance and Payment Information
We accept most insurance plans including Medicare. Co-payments are
required at the time of your visit. If you are a self paying patient,
payment is expected in full at the time of service. We accept cash,
checks, VISA, MasterCard and Discover.
Prescriptions Refills
Patients are responsible for monitoring prescription refills. Please
make the physician and nurse aware of the need for refills at the time
of your visit. If you are not scheduled for an appointment but need a
prescription refill, please call the office during business hours and
leave a message for your nurse. We request 48 hours to process refill
requests.
Please remember that in certain situations appropriate medical care requires the benefit of a patient examination before a prescription can be filled.
