Admission to the Hospital
If you require admission to the hospital, your doctor may order specific tests or procedures and you will have to register either in person or by telephone.
You may call our pre-registration number at (606) 833-3771 to provide your information.
To register in person, please report to the hospital at least 30 minutes prior to any scheduled test. You may report to the registration desk located near the Emergency Department.
What You Will Need to Register- You will be asked a series of questions by one of our registration clerks. Please be prepared to provide:
- Your name, address, telephone number and social security number for both you and your spouse if you are married.
- A current health insurance card; if you do not have and insurance card, but have insurance, please provide.
- Name, address and telephone number of you and your spouse's employer;
- Medicare card (if eligible)
- Public assistance card (if eligible)
- Your physicians name and any written orders that you were given by the physician.
